  3. Sign Informed Consent— this is HIPAA and Office Policies

  4. Sign Informed Consent for Telepsychology Services— this is a separate one that needs to be completed in addition for doing telepsych services. 

  5. Insurance— add policy holder’s name, insurance plan, ID number, group number, effective date, policy holder’s date of birth and address
